PETTY OFFICER SAMPSON'S BOLD LEADERSHIP, SOUND JUDGEMENT AND COMPLETE DEDICATION TO DUTY REFLECTED CREDIT UPON HIM AND WERE IN KEEPING WITH HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICES.

WHILE SERVING AS TCCC-AC PROGRAM MANAGER FOR MEDICAL SKILLS TRAINING CENTER, HE MANAGED 35 TCCC COURSES, PROVIDING OVER 800 HOURS OF TRAINING, RESULTING IN THE CERTIFICATION OF 487 MARINES AND SAILORS ACROSS 23 III MEF COMMANDS. IN ADDITION, AS BLS TRAINING SITE FACULTY, HE INSTRUCTED 25 BLS PROVIDER COURSES AND THREE INSTRUCTOR COURSES, PRODUCING 240 QUALIFIED BASIC LIFE SUPPORT CERTIFICATIONS. MOREOVER, AS AN EXPERIENCED DEPLOYER, HE CONDUCTED TRAINING DURING THREE PACOM AOR EXERCISES WHICH INCREASED HOST NATIONS RELATIONS, PREPAREDNESS, PROFICIENCY, AND INTEROPERABILITY IN SUPPORTING COMBAT OPERATIONS. AS ACTING SENIOR LINE PLATOON CORPSMAN DURING THE INITIAL TRAINING PHASE AND DEPLOYMENT, PETTY OFFICER DAEL DEMONSTRATED PROFESSIONAL MEDICAL KNOWLEDGE AND COMPETENCE BY ADMINISTERING OVER 400 IMMUNIZATIONS, ENSURING ONE HUNDRED PERCENT OF LIMA COMPANY'S 167 MARINES WERE DEPLOYABLE. ADDITIONALLY, HE CONDUCTED 11 MEDICAL COVERAGES AND MINIMIZED INJURIES DURING PRE DEPLOYMENT. MOREOVER, WHILE DEPLOYED TO AFGHANISTAN, HM2 DAEL MANAGED THE TREATMENT AND CARE OF 11 PATIENTS MEDICALLY EVACUATED TO CAMP SHORAB AND ENSURED THEY WERE TRANSFERRED TO A HIGHER ECHELON OF CARE IN A TIMELY MANNER. FINALLY, PETTY OFFICER DAEL'S MENTORSHIP HELPED NINE JUNIOR CORPSMEN AND PEERS BECOME QUALIFIED AS FMF CORPSMEN THROUGH A VARIETY OF CONTINUING CLASSES AND BOARDS. PETTY OFFICER DAEL'S ENTHUSIASM, PROFESSIONALISM AND DEVOTION TO DUTY REFLECTED CREDIT UPON HIM AND WERE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.
AS A FLEET MARINE FORCE CORPSMAN, 3D MARINE LOGISTICS GROUP, III MARINE EXPEDITIONARY FORCE FROM AUGUST 2012 TO SEPTEMBER 2014. DURING THIS PERIOD, PETTY OFFICER ZARATE PERFORMED HIS DEMANDING DUTIES IN AN EXEMPLARY AND HIGHLY PROFESSIONAL MANNER. WHILE ASSIGNED TO REGIMENTAL MEDICAL AUGMENTATION TEAM, HE WAS THE SOLE HM FOR AMMO COMPANY WHERE HE PERFORMED NEARLY 300 HOURS OF ACUTE CARE FOR HIS OVER 100 MARINES. ALSO RESPONSIBLE FOR HIS COMPANY'S COMPLETE MEDICAL READINESS, HE PERFORMED 330 PERIODIC HEALTH ASSESSMENTS (PHA) AND OVER 500 VACCINATIONS IN ORDER TO INCREASE READINESS TO AN ACCEPTABLE LEVEL. THESE INTERVENTIONS RAISED THE COMPANY'S FMR PERCENTAGE FROM 82.5 PERCENT TO 93.4 PERCENT, EXCEEDING THE STANDARDS SET FORTH BY BUMED. ADDITIONALLY, HE PARTICIPATED IN THE 31ST MEU'S CERTEX WHERE HE WAS THE SENIOR SHORE HM IN THE SIMULATED BOAT RAIDS. HE ALSO ASSISTED IN THE COORDINATION AND EXECUTION OF TWO CASUALTY EVACUATION DRILLS ENABLING THE MEU TO ENSURE ITS WORLDWIDE DEPLOYABILITY IN SUPPORT OF III MEF OPERATIONS. FINALLY, AS GENERAL DUTY HM AT EVANS CLINIC, HE ASSISTED IN THE DIAGNOSES AND TREATMENT PLANS OF OVER ONE THOUSAND PATIENTS, AND RECORDED AND FORWARDED CRITICAL PATIENT UNIT DATA WHICH ASSISTED THE COMMANDING OFFICER IN DETERMINING THE BATTALION'S MEDICAL SUPPORT REQUIREMENTS. PETTY OFFICER ZARATE'S MOTIVATION, INITIATIVE, AND PROFESSIONALISM REFLECTED CREDIT UPON HIMSELF AND WERE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.
AS FLEET MARINE FORCE CORPSMAN, GOLF COMPANY, 2D BATTALLION, 2D MARINE REGIMENT, 2D MARINE DIVISION, II MARINE EXPEDITIONARY FORCE FROM AUGUST 2012 TO OCTOBER 2014. PETTY OFFICER QUITO'S STEADFAST DEDICATION TO PROVIDING HIGH QUALITY MEDICAL CARE WAS ESSENTIAL IN ASSISTING FOUR MEDICAL PROVIDERS WITH THE TRIAGE, TREATMENT, AND DISPOSITION OF 350 SICK-CALL ENCOUNTERS. IN ADDITION, HIS COMMITMENT TO MISSION ACCOMPLISHMENT WAS INSTRUMENTAL IN SUSTAINING A 98% FULLY MEDICAL READY STATUS FOR 150 MARINES, EXCEEDING 5TH AND 6TH FLEET REQUIREMENTS, BY ADMINISTERING 225 IMMMUNIZATIONS, 70 PERIODIC HEALTH ASSESSMENTS AND 125 PRE-DEPLOYMENT SCREENINGS WHILE ASSIGNED TO BATTALION LANDING TEAM 2/2 IN SUPPORT OF THE 22D MARINE EXPEDITIONARY UNIT. HIS VAST MEDICAL KNOWLEDGE AND ABILITY TO INSTRUCT FOREIGN NATIONALS WAS CRUCIAL TO THE SUCCESS OF THE BLACK SEA ROTATIONAL FORCE 2013 DEPLOYMENT AS HE PROVIDED 60 HOURS OF TACTICAL COMBAT CASUALTY CARE AND COMBAT LIFE SAVER COURSES TO 140 MARINES AND MULTI-NATIONAL FORCES AND PROVIDED MEDICAL COVERAGE FOR 70 MULTI FORCE TRAINING AND LIVE FIRE EVOLUTIONS, DIRECTLY CONTRIBUTING TO IMPROVED RELATIONS WITH ROMANIA AND THE REPUBLIC OF GEORGIA. PETTY OFFICER SECOND CLASS MALEHORN CONSISTENTLY PERFORMED HIS DEMANDING DUTIES IN AN EXEMPLARY AND HIGHLY PROFESSIONAL MANNER. DEPLOYED IN SUPPORT OF OPERATION ENDURING FREEDOM, HM2 MALEHORN COMPLETED OVER 150 SECURITY PATROLS AND CRITICAL RESUPPLY MISSIONS, AND TREATED 50 MARINES, 30 ANA, AND 15 LOCAL NATIONALS. ADDITIONALLY, HE DILIGENTLY LEAD HIS TEAM AND CONDUCTED 30 SHOTEX RESULTING IN REDUCING MEDICAL DEFICIENCIES BY 87%, VASTLY IMPROVING THE UNIT LEVEL OF READINESS. COMMENDATION FOR HEROISM WHILE SERVING AS FLEET MARINE FORCE CORPSMAN WITH THE 26TH MARINE EXPEDITIONARY UNIT, COMPANY BRAVO, AFGHANISTAN. PETTY OFFICER DAVID, DURING AN AMPHIBIOUS INSERTION ON 13 NOVEMBER 2001 ACTED WITH HEROISM AND VALOR BY TAKING IMMEDIATE ACTION AS AN IED DETONATED, IMMEDIATELY KILLING FIVE AND SEVERELY INJURING NINE OTHER MARINES. PETTY OFFICER DAVID TOOK SWIFT ACTION, DEPLOYING LIFESAVING TECHNIQUES AND MEASURES WHILE ENDANGERING HIMSELF TO SAVE THE LIVES OF HIS FELLOW BROTHERS IN ARMS, IN THE WAKE OF PURE TERROR. IT IS BECAUSE OF PETTY OFFICER DAVID'S SWIFT ATTENTION THAT THE NINE MARINES SURVIVED. THE ACTIONS OF PETTY OFFICER DAVID ATTEST TO HIS SINCERE COMMITMENT TO OTHERS REGARDLESS OF THE SITUATION.
