PROFESSIONAL ACHIEVEMENT IN THE SUPERIOR PERFORMANCE OF HIS DUTIES WHILE SERVING AS SENIOR LINE CORPSMAN, FOX COMPANY, 2ND BATTALION, 9TH MARINES UNIT DEPLOYMENT PROGRAM FROM AUGUST 2014 THOUGH FEBRUARY 2015 AND SENIOR LINE CORPSMAN, GOLF COMPANY, 2ND BATTALION, 6TH MARINES DURING THE 26TH MEU FROM SEPTEMBER 2015 THROUGH MARCH 2016. DURING THIS PERIOD, PETTY OFFICER JAMESON WAS APPOINTED OVER AND SUPERVISED 18 JUNIOR SAILORS IN THE ADMINISTRATION OF 3,700 IMMUNIZATIONS, 372 PHA'S, AND THE HEALTH CARE OF 322 MARINES AND SAILORS, MAINTAINING A 98% MEDICAL READINESS. ADDITIONALLY, WHILE TRAINING IN THE PHILIPPINES DURING UDP, HE WAS INSTRUMENTAL IN THE TRIAGE, CARE AND TRANSPORTATION OF 13 HEAT CASUALTIES ON TWO SEPARATE MASS CASUALTY INCIDENTS TO A HIGHER ECHELON OF CARE. DUE TO HIS KNOWLEDGE AND ABILITIES, ALL MARINES WERE RETURNED TO FIT FOR FULL DUTY. MOREOVER, AS A CLS INSTRUCTOR, HE COORDINATED 12 CLS COURSES CERTIFYING 366 MARINES AS COMBAT LIFE SAVERS. FURTHER, DURING A BILATERAL TRAINING EXERCISE IN OMAN, HE COORDINATED AND TAUGHT SIX COMBAT LIFE SAVER CLASSES TO 130 OMANI SOLDIERS, ENHANCING THE RELATIONS BETWEEN COUNTRIES. FINALLY, DUE TO HIS MENTORSHIP, 27 OF HIS JUNIOR SAILORS EARNED THEIR FMF WARFARE DEVICES, EIGHT EARNED THEIR SURFACE WARFARE QUALIFICATION, AND SEVEN EARNED THEIR AVIATION WARFARE QUALIFICATIONS. BY HIS EXCEPTIONAL ABILITY, PERSONAL INITIATIVE AND TOTAL DEDICATION TO DUTY, PETTY OFFICER JAMESON REFLECTED CREDIT UPON HIMSELF AND UPHELD THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.
Professional Achievement in the superior performance of his duties while serving as Line Corpsman, Company E, 2D Battalion 6TH Marine Regiment, 2D Marine Division from 10 JUNE 2017 to 01 JULY 2020. Hospitalman Third Class performed his duties in an exemplary and highly professional manner. While forward deployed as part of the 26th Marine Expeditionary Unit and Special Purpose Marine Air and Ground Task Force- Crisis Response- Africom, conducted Theater Security Cooperation Training with four nations during five large scale exercises, reinforcing key partnerships with foreign allies. He instructed 83 Foreign Military members in Combat Life Saving techniques, directly influencing more positive relationships between partner forces and their Marine counterparts. He served as Senior Line corpsman twice for 10 months, successfully training and turning over with senior Corpsman after achieving 100% readiness prior to SPMAGTF deployment. He served as a mentor to the Marines in his platoon and Company, training them ceaselessly in his field of expertise to include certifying 275 Marines in Tactical Combat Casualty Care, and ensuring medical readiness in the creation and supervision of five dental blocks, four pre-deployment health assessment and audiogram blocks, five post-deployment health assessment and audio blocks, supervised 600 immunizations, and 320 periodic health assessments. By his steadfast leadership, tactical proficiency, and total dedication to duty, Hospitalman Third Class Gomez reflected credit upon himself and upheld the highest traditions of the United States Naval Service.
Sailor is a line corpsman for Alpha Co. 2nd platoon where during the time spent in the BAS of January to May 2019 he helped bring the medical readiness from 12% to 100%, created skeleton records for over 130 deploying marines, printed and inserted chronological records of medical care, DD2766, and SF600 into the skeleton records, performed HIV blood draws on A Co. marines and documented all blood draws onto MRSS.
Right afterwards, instructed Marines to write down EDIPI, Last names, and vaccinations into the green log book, where it was inputted into MRRS shortly afterwards, instructed all A Co. female Marines to do HCG Pregnancy tests, helped inventory A Co. vaccinations, and created patient immunization encounters for the IDC of the BAS.
For field ops/coverages in the months of January to May 2019, the sailor participated in Steel Night in Ft. Irwin, two track run field ops, one beach field op, six mile and nine mile hike coverage, A Co. warriors night, and one beach field op all for which account for which account over twenty days spent with marines for medical coverage before a UDP deployment.
During the beginning of the UDP, sailor did daily sick call and helped do a beach clean up where he received a thank you letter from the mayor of a neighbouring town in Okinawa. From the time of 4 July to September 2019 sailor was the sole corpsman for 2nd platoon, where he participated and did medical coverage in an CMP Shoot and Grenade throwing, stood duty at the Camp Fuji BAS, helped direct marines from Camp Fuji to naval hospital Yokosuka for medical referrals, performed minor procedures, performed sick call, did medical coverage in a two day AAV CMP shoot, and medical coverages for MCMAP.
After Sailor was in Okinawa again in time of September to November 2019 he helped with daily sick call and helped record and input all shot safety stand downs into MRSS. Since returning from deployment, Sailor has reviewed Marines' online PHAs, printed and replaced out-of-date Audiograms, chronological records of medical care, DD2766, and SF600 with current ones and inputed them into the original records, done daily sick call, participated in a one day AAV convoy, eight day ship op on the USS Portland where he received a letter of appreciation and ship coin from the sickbay Chief, and a four day AAV CMP shoot with the Japanese ground self defense force.
PROFESSIONAL ACHIEVEMENT IN THE SUPERIOR PERFORMANCE OF HIS DUTIES WHILE SERVING AS A LINE CORPSMAN FOR 1ST PLATOON, FOX COMPANY, 2ND BATTALION, 9TH MARINE REGIMENT FROM XXXXXXX TO XXXXXXX. PETTY OFFICER DOUGLAS WAITE PARTICIPATED IN MORE THAN 80 DISMOUNTED PATROLS DURING WHICH HE EXPERTLY PROVIDED MEDICAL CARE TO NUMEROUS MARINES, IRAQI COMBAT ENGINEERS, AND IRAQI CITIZENS IN SUPPORT OF OPERATION IRAQI FREEDOM. HM3 WAITE CONTINUED TO DEMONSTRATE SUPERIOR PERFORMANCE OF HIS DUTIES WHILE SERVING AS SENIOR LINE CORPSMAN FOR FOX COMPANY, 2D BATTALION, 9TH MARINE REGIMENT WHILE IN SUPPORT OF SECURITY COOPERATION MARINE AIR GROUND TASK FORCE LEADING TO HIS SELECTION AS SAILOR OF THE QUARTER. UPON TRANSFER TO 2D TANK BATTALION, PETTY OFFICER WAITE WAS ENTRUSTED WITH ALL SUPPLY RESPONSIBILITIES FOR THE BATTALION'S AID STATION WHEREIN HE MANAGED AN ANNUAL BUDGET IN EXCESS OF $160,000 AND MADE MULTIPLE CHANGES TO IMPROVE EFFICIENCY IN THE AID STATION'S INVENTORY AND ORDERING PROCEDURES. BY HIS EXCEPTIONAL ABILITY, PERSONAL INITIATIVE AND TOTAL DEDICATION TO DUTY, PETTY OFFICER WAITE REFLECTED CREDIT UPON HIMSELF AND UPHELD THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.
WHILE SERVING AS SENIOR LINE CORPSMAN, COMPANY A, FIRST BATTALION, THIRD MARINES, THIRD MARINE REGIMENT, THIRD MARINE DIVISION ON 12 APRIL 2018 DURING MOUNTAIN EXERCISE 2-18. ON THIS DAY, HM3 VILLAREAL RESPONDED TO A MARINE THAT WAS DISPLAYING SIGNS OF SERIOUS MEDICAL COMPLICATIONS. HM3 VILLAREAL QUICKLY ASSESSED THE MARINE, RECOGNIZED SIGNS OF SEVERE HYPOTHERMIA, AND DETERMINED THE SITUATION TO BE LIFE THREATENING. HE QUICKLY INFORMED HIS CHAIN OF COMMAND OF THE SEVERITY OF THE SITUATION AND WITHOUT HESITATION, IMMEDIATELY BEGAN ADMINISTERING EMERGENCY FIRST AID AND STABILIZED THE MARINE FOR OVER NINETY MINUTES UNTIL AN EMERGENCY MEDICAL TEAM WAS ABLE TO ARRIVE ON SCENE. CORPORAL VILLAREAL'S INITIATIVE, PERSERVERENCE, AND TOTAL DEDICATION TO DUTY REFLECTED CREDIT UPON HIM AND WERE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE MARINE CORPS AND THE UNITED STATES NAVAL SERVICE.
HIS SWIFT ACTIONS AND MEDICAL KNOWLEDGE DURING A COMPOSITE TRAINING UNIT EXERCISE (COMPTUEX) ABOARD USS FORT MCHENRY FROM XXXXXXX TO XXXXXXX WAS A DIRECT ACTION OF TRIAGING, MEDEVACING, AND PROVIDING EXPEDIENT FIELD MEDICAL CARE TO THREE MARINES AFTER AN AAV WRECK. AS SENIOR LINE CORPSMAN OF HIS PLATOON, HE WAS RESPONSIBLE FOR MAINTAINING OVER 96% MEDICAL READINESS FOR A PLATOON OF 35 MARINES. ADDITIONALLY, DURING A MEDICAL EMERGENCY ABOARD THE USS FORT MCHENRY, CORPORAL VILLAREAL RAPIDLY RESPONDED TO A DOWNED MARINE, PROVIDING MEDICAL CARE, AND APPROPRIATE ACTIVATION OF THE SHIP BOARD MEDICAL RESPONSE TEAM, RESULTING IN THE LIFE SAVING INTERVENTION FOR A MARINE WITHIN HIS PLATOON. CORPORAL VILLAREAL'S INITIATIVE, PERSERVERENCE, AND TOTAL DEDICATION TO DUTY REFLECTED CREDIT UPON HIM AND WERE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE MARINE CORPS AND THE UNITED STATES NAVAL SERVICE.
HEROIC ACHIEVEMENT AS SENIOR LINE CORPSMAN FOR BRAVO COMPANY, 2D TANK BATTALION. RECEIVED FOR HEROIC ACTION ON MAY NINTH, TWO THOUSAND AND NINE. PETTY OFFICER CARTER, FRANK J. ACTED WITHOUT HESITATION AND ABOVE THE CALL OF DUTY DURING A FIREFIGHT IN HIT, IRAQ WHILE CONDUCTING A ROUTINE DISMOUNTED IED CLEARANCE PATROL TO RESCUE TWO STRANDED IRAQI CHILDREN AND SAVE THE LIVES OF TWO FELLOW MARINES. HM3 CARTER, UNDER HEAVY ENEMY SMALL ARMS FIRE, EXPOSED HIMSELF SEVERAL TIMES TO BRING AID TO THE WOUNDED AND EVACUATE THE CRITICALLY INJURED. HM3 CARTER IS CREDITED WITH KILLING THREE ARMED INSURGENT FIGHTERS AND CALLING IN TWO MEDAVACS THAT SAVED THE LIVES OF FOUR PEOPLE. PETTY OFFICER CARTER'S EXCEPTIONAL PROFESSIONALISM, UNRELENTING PERSEVERANCE, AND LOYAL DEVOTION TO DUTY REFLECTED CREDIT UPON HIM AND WERE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.
FOR PROFESSIONAL ACHIEVEMENT IN THE SUPERIOR PERFORMANCE OF HIS DUTIES WHILE SERVING AS CORPSMAN, 5TH MARINES, 1ST MARINE DIVISION FROM 22 JUNE 2009 TO 15 AUGUST 2012 IN SUPPORT OF OPERATION ENDURING FREEDOM. PETTY OFFICER JASON ELLIS CONSISTENTLY PERFORMED HIS DUTIES IN AN EXEMPLARY AND HIGHLY PROFESSIONAL MANNER, INSPIRING HIS PEERS WITH OUTSTANDING OPERATIONAL AND GARRISON TREATMENT AND PROVIDING EDUCATION TO ALL SAILORS AND MARINES WITHIN HIS SCOPE OF CARE. AS A LINE CORPSMAN, FOX COMPANY, 2ND BATTALION, 5TH MARINES, DURING OPERATION MOSHTARACK IN MARJAH, AFGHANISTAN, HE PROVIDED EXCEPTIONAL MEDICAL CARE TO BOTH HIS COMBAT WOUNDED MARINES AND LOCAL NATIONALS, EXEMPLIFYING THE ROLE OF THE CORPSMAN IN THE BATTLESPACE. PETTY OFFICER ELLIS' STERLING EXAMPLE AND UNSWERVING DEVOTION TO DUTY REFLECTED GREAT CREDIT UPON HIM AND WERE KEEPING WITH THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.
