PROFESSIONAL ACHIEVEMENT AS GENERAL DUTY CORPSMAN, SUBSTANCE ABUSE REHABILITATION PROGRAM, DIRECTORATE FOR MENTAL HEALTH, NAVAL MEDICAL CENTER, SAN DIEGO, CALIFORNIA FROM MARCH 2011 TO APRIL 2013. HM2 ESTEVEZ MANAGED ALL MEDICAL SUPPORT FOR FOUR PROVIDERS WHO PROVIDED CARE FOR MORE THAN 600 PATIENTS. PETTY OFFICER ESTEVEZ COLLECTED 1123 LAB SPECIMENS TO INCLUDE THE ARMED SERVICES' FIRST TOXICOLOGY SCREENING FOR SYNTHETIC CANNABINOID METABOLITES, UPDATED AND PREPARED 560 MEDICAL RECORDS, PERFORMED 121 INITIAL CORPSMAN INTAKES, AND COMPLETED 188 INDIVIDUAL DISCIPLINARY TEAM UPDATES. IN ADDITION, HE WAS INSTRUMENTAL IN NMCSD'S MEDICAL INSPECTOR GENERAL (MEDIG) AND JOINT COMMISSION FULL ACCREDITATION. PETTY OFFICER ESTEVEZ'S PROFESSIONALISM, PERSONAL INITIATIVE AND RELENTLESS DEVOTION TO DUTY REFLECTED CREDIT UPON HIM AND WERE IN KEEPING WITH THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.




FOR PROFESSIONAL ACHIEVEMENT AS LEADING CHIEF PETTY OFFICER, SUBSTANCE ABUSE REHABILITATION PROGRAM DEPARTMENT, DIRECTORATE OF MENTAL HEALTH, NAVAL MEDICAL CENTER SAN DIEGO, CALIFORNIA FROM JUNE 2010 TO OCTOBER 2013. CHIEF PETTY OFFICER THOMAS LED 34 ENLISTED AND 28 CIVILIAN COUNSELORS IN DAILY CLINICAL OPERATIONS. HE PROVIDED SUPERB TREATMENT FOR 212 PATIENTS, LOGGED 2,000 HOURS OF PATIENT CARE, CONDUCTED 122 INTAKE ASSESMENTS AND 120 SCREENINGS, ENHANCING COPING SKILLS AND METHODS OF INTERVENTION AND ENABLING SERVICE MEMBERS TO RETURN TO THEIR UNITS FIT FOR DUTY. ADDITTIONALLY, AS COMMAND CAREER COUNSELOR, HE IMPLEMENTED A NEW CAREER INFORMATION MANAGEMENT PROGRAM DESIGNED TO TRACK CAREER DEVELOPMENT PROGRESSION, RESULTING IN A 30 PERCENT INCREASE IN READINESS. CHIEF THOMAS' SUPERIOR PERFORMANCE OF DUTY DURING THIS TOUR IS THE HALLMARK OF A CAREER DEVOTED TO ACCOMPLISHING BROAD AND DIVERSE TASKS AND HIGHLIGHTS THE CULMINATION OF TWENTY-TWO YEARS OF HONORABLE AND DEDICATED NAVAL SERVICE.




MERITORIOUS SERVICE AS PROGRAM DIRECTOR OF THE BAHRAIN SUBSTANCE ABUSE REHABILITATION PROGRAM, U.S. NAVAL HOSPITAL SIGONELLA FROM DECEMBER 2014 TO AUGUST 2015. AS THE SOLE SUBSTANCE ABUSE REHABILITATION PROGRAM COUNSELOR IN THE FIFTH FLEET AREA OF OPERATIONS, PETTY OFFICER KELLER CONSISTENTLY PERFORMED HIS DEMANDING DUTIES IN AN EXEMPLARY AND HIGHLY PROFESSIONAL MANNER. HE PROVIDED OVER 800 HOURS OF DIRECT PATIENT CARE IN THE FORM OF SCREENINGS, PROSPECTIVE COUNSELOR INTERVIEWS, GROUP AND INDIVIDUAL COUNSELING SESSIONS, WHILE SIMULTANEOUSLY MANAGING AN OUTPATIENT LEVEL I TREATMENT PROGRAM FOR 5TH FLEET. THE IMPLEMENTATION OF THE LEVEL I TREATMENT PROGRAM SAVED TENANT COMMANDS AT LEAST $10,000 IN TRANSPORTATION AND LODGING, AND DUTY TIME LOST DUE TO TAD ASSIGNMENTS. PETTY OFFICER KELLER'S EXCEPTIONAL PROFESSIONALISM AND SELFLESS DEVOTION TO DUTY REFLECTED CREDIT UPON HIMSELF AND UPHELD THE HIGHEST TRADITIONS OF THE UNITED STATES NAVAL SERVICE.
